Important Insurance Information
In order to make your visit to our office as smooth as possible, here are a few things to keep in mind.
ELIGIBILITY
It is the patient’s responsibility to provide their current/active insurance at time of scheduling.

PRE-AUTHORIZATION
Please keep in mind that it is the patient and referring physician’s responsibility to confirm pre-authorization requirements and obtain the approval that is required for the exam.
Patients should contact us two days before their appointment to confirm that their exam has been approved.
