Insurance Pre-Authorization Program

 

For your convenience, we can process your patient's pre-authorization paperwork. Simply fax your patient’s information on the Pateint Referral Form, and we will obtain the necessary pre-auth information, schedule the exam and provide a confirmation status by the end of the day.
